    Instructions: This questionnaire should be submitted to the district secretary by the deadline established by Board of Ministry. The Board of Ministry will use it to consider your request for ordination or recognition. If recommended to the District Assembly of Ordination or Recognition, this questionnaire is presented to the presiding General Superintendent for his/her review and is filed with the General Secretary's office following district assembly. Be sure to read all questions carefully and be sure all pertinent questions are completed. If you have any questions regarding this questionnaire contact your district secretary or Board of Ministry/Credentials Board Chairperson.
