Use this form to describe your proposed pollution prevention projects for the summer Southwest Ohio Pollution Prevention (P2) Internship Program. The project selection criteria are provided to guide the District in evaluating your projects. IMPORTANT: Prior to completing this application, please determine if your company is willing to share project outcomes with the district in the form of a written case study. Confidential and proprietary information will not be disclosed.

Project Description and Expected Deliverables
Describe your sustainability projects in detail and what deliverables you would expect from the intern upon completion. Indicate the company supervisor’s role as it relates to this project and what your company’s long-term commitment to sustaining recommended results from the intern program. *

Internship Setting
Please describe the percentage of time you expect the intern to work onsite vs. remotely, the resources you’ll provide the intern vs. what the intern will be expected to have (i.e. a laptop, transportation), and where the intern will be working if onsite. Please also include the hourly wage or rage you intend to offer the intern.*
