1. Voluntary Participation

I acknowledge that I am volunteering my time and services to assist One Small Act Miami County (“the Organization”).

I understand this is a volunteer role and not employment, and participation is completely voluntary.

2. Assumption of Risk

I understand that volunteer activities may involve physical activity and interaction with other volunteers and community members. These activities may include, but are not limited to:

Lifting or carrying supplies
Sorting and organizing donations
Participating in community outreach events
Packing food or resource bags
Working around other volunteers and community guests

I voluntarily assume all risks associated with volunteering.

3. Liability Release

In consideration of being permitted to volunteer, I release and hold harmless One Small Act Miami County, its organizers, volunteers, sponsors, and affiliates from liability for any injury, illness, or damages that may occur as a result of my participation.

4. Health & Medical

I certify that I am physically able to participate in volunteer activities.

I understand I am responsible for my own medical insurance and expenses.

5. Code of Conduct

I agree to:

Treat all individuals with dignity, kindness, and respect
Maintain privacy and confidentiality of anyone served
Follow all safety instructions and organizational guidelines

6. No Compensation

I understand I am volunteering and will not receive compensation, wages, or benefits for my time.
