This form is for Match Alteration Requests for the 2026 season.

All requests require the details of the match as currently scheduled and the requested details for the match.

Requests must be lodged at least 3 business days prior to the kick-off date; any requests submitted later will not be processed.

Please note that the final decision on your request is made by CF. This decision is final and not subject to appeal.
