To withdraw you must submit this form by the 7th day of the month you wish to withdraw (i.e. give notice on Dec. 7th for a Dec. 31st last day of class):

- Continue with your class/lessons
- Have a credit added to your account for next month. This can be used for any future programs and does not expire
- Donate your tuition to our scholarship fund for those familes in need
