Who needs to submit a Health Form?

• Any child with cancer.
• Any sibling who is attending SUMMER CAMP.
• Any child with sickle cell.
• Any staff member (paid or volunteer).

What type of Health Form do I need to submit?

• Children who have or had cancer
  • Camper Health Form (required)
• Siblings attending Summer Camp:
  • Sibling Health Form or a general physical exam form (like the one submitted for school)
• Children with sickle cell:
  • Sickle Cell Health Form

Please note: all campers and siblings must use Happiness Is Camping's forms. School paperwork and general physical exam forms will no longer be accepted.
