  • This online Green Form consists of two parts.

    Part 1 Club Section.

    The secretary completes this part and by using the "submit form" button this part of the form will be send to the Demonstrator/Judge/Teacher/Speaker. On the email send to the Demonstrator/Judge/Teacher/Speaker, will be a link to open up their part of the form.

     

    Part 2 Demonstrator/Judge/Teacher/Speaker Section

    The Demonstrator/Judge/Teacher/Speaker will receive an email containing a link to open their part of the Green Form. (See explanation in the section above)

    The Demonstrator/Judge/Teacher/Speaker, completes Part 2 and by using the "submit form" button, sends this part to the secretary, Green Form is complete.

    Both the secretary and the Demonstrator/Judge/Teacher/Speaker will receive copies of both Part 1 and Part 2 of the form.

    Print Forms

    You can print the forms by clicking on the "Print Form" button at the bottom of the form, but the better option for printing a hard copy of your entry is printing the confirmation email you will receive after submitting the form. It contains all the info but is much more compact.

  • Secretary, remember to send a reminder email to the Demonstrator/Judge/Teacher/Speaker two weeks before the date of the event.

    Book this into your Google/Outlook or any other calender you might be using now.

  • In the next box you are asked to enter the venue's eircode, if you do not know the eircode use this eircode finder 

  • Upon submission both the Club Secretary and Demonstrator/Judge/Teacher/Speaker will receive an email showing the details entered on this form. The Demonstrator/Judge/Teacher/Speaker will also receive a separate email with a link to their part of the Green Form.
