Media and Data Collection
Data Collection OVERVIEW

Skyland Workforce Center consists of a group of DC nonprofit organizations that work together to help DC residents to acquire skills, obtain and retain employment, and improve their quality of life. Skyland manages a shared database system that can only be accessed by partner agencies. The shared database system enables partner organizations to, among other things:
- Serve DC adults more effectively and efficiently by facilitating collaboration between partner agencies with the goal of helping DC adults acquire job skills and education.
- Coordinate and integrate services in a manner that will help participants increase their likelihood of achieving goals and success.
- Help facilitate participant referrals between CIP partner organizations.
- Reduce the amount of time a participant spends completing intake forms and repeating similar information from one organization to the next.
The collaborative has a standard intake form that is used by all partner organizations to collect basic information about you. This basic information will be shared via the database with Skyland partner organizations. Partners will use the shared data to help coordinate the programs and services you need to succeed. In addition, each partner organization may collect additional information from you that will not be shared with other agencies, such as sensitive health information or case management notes. Neither Skyland nor partners will share any information you give to us with non-Skyland/CIP partner organizations without your explicit permission.

MEDIA RELEASE
The Skyland Workforce Center and partner organizations ask for your permission to use your likeness (images or recordings) to promote the work of partners in the community. Photos and images may be used on items such as printed materials (brochures), websites, or fundraising materials.
I hereby grant the Skyland and partner organizations and their respective licensees, successors and assigns, the right and permission, with respect to those videotapes and/or photography taken of me or the minor named below on whose behalf I am signing, and with respect to any printed matter in connection therewith, to do the following:
- To include such images in all editions of the product tentatively entitled “promotional flyer” and/or on its product cover, in all media, and in the advertising, publicity, and promotion thereof.
- To use my name, or the name of the minor on whose behalf I am signing, in connection with foregoing.
- Thereby release, discharge, and agree to indemnify and hold harmless the Skyland and partners and their respective heirs, legal representatives, licensees, successor and assigns, from all claims and demands whatsoever arising out of or in connection with the foregoing, and waive any right to inspect or approve the same.
