Application Instructions
Thank you for your interest in applying for funding from the United Way of Palo Pinto County. Please read and follow the instructions carefully. Our review team relies on clear, concise, and complete information to evaluate your request fully and fairly.

1. Application Deadline:
All application materials must be submitted online by midnight on Wednesday, December 31, 2025.
Late or incomplete applications will not be considered.
2. Application Packet Requirements:
Before you begin the webform, please download all necessary forms from this site or our website.
We recommend gathering and completing all materials in advance. This website has auto save, however it is always best and highly recommended to type your answers in a non-web based file as you go to ensure no information is lost.
Uploaded files must be in one of the following formats: .doc, .docx, .xls, or .pdf (PDF is preferred).
Your complete application packet must include the following:
a) Application Form (Online)
Please be clear, concise, and answer all questions in the order presented.
b) Proposed Program/Project Budget
Please use the Grant Program Budget Worksheet provided. YOU MUST download a copy of this form to your computer prior to entering our infomation in the form. If the form is not copied and downloaded to your computer, your infomation will not be saved. Download Here
c) Financial Statements
Include a balance sheet and profit/loss statement for the agency’s most recently completed fiscal year.
Combine both into one document.
e) IRS Form 990
Submit a complete copy of your latest IRS Form 990.
f) Audit or CPA Review (If Available)
If available submit a copy of your audit or CPA financial review corresponding to the same fiscal year as your latest Form 990.
g) Human-Interest Stories
Provide two current stories that highlight the impact of your agency’s work.
These may be used for publicity and should be compiled into one document.
h) Compliance Statements
Include a signed copy of the Anti-Terrorism Compliance Statement and the Non-Discrimination Policy Statement. Download Here
3. Agency Agreements
Agencies approved for funding will receive an Agency Agreement for review and signature. Funds will not be distributed until the agreement has been signed and returned.
EVALUATION CRITERIA
Funds Distribution Committee members will use the following criteria for reviewing proposals.
40% COMMUNITY NEED
Does the program address a recognized health and human service need in our community? Is the need consistent with UWPPC mission and Funding Priorities? How serious is the problem?
30% IMPACT
How well does the program demonstrate a meaningful linkage between community needs, program activities and outcomes? Will the program provide a meaningful volume of services and/or people served? Will United Way funding make a difference?
20% ABILITY AND EVALUATION
Does the agency have a history of reliability? Are there adequate staff and resources to conduct this program? Does the program plan seem sound? Are clear goals and objectives written? Are measurable outcomes evident? How well does the agency/program demonstrate the ability to deliver and measure proposed outcomes?
10% FINANCIAL MANAGEMENT
Is the financial information presented clearly & accurately? Does the agency have a balanced budget? Does the agency have adequate reserves? Is there diversified funding? Is other funding available? Will funds requested from UWPPC support direct client services? Are overhead expenses a reasonable % of total?
TRACK RECORD (For 2025 Funded Partners only)
Grade: A, B, C, D, E, F Did the agency accomplish its goals and objectives last year? Did the agency make a difference with last year’s United Way funds?
