1. I have read and agree with the Statement of Faith of the Morristown Christian Homeschool Co-op.

2. It is my responsibility to stive diligently toward observance of the Code of Conduct as God enables me by the power of the Holy Spirit.

3. I give permission for MCHC to contact my child's previous school/co-op (if applicable) for any information deemed necessary.

4. I agree to pay all financial obligations to MCHC in a timely manner.

5. The MCHC reserves the right to dismiss any student who does no cooperate with the educational process.
