  • Welcome to the Year of Moment Program!  

    This agreement outlines the program and coaching relationship.

    Please read through it carefully and sign where requested.

     

    Coaching is a partnership (defined as an alliance, not a legal business partnership) between the Coach and the Client in a thought-provoking and creative process that inspires the client to maximize personal and professional potential. The International Coach Federation (ICF) defines coaching as the following:

     "Professional Coaching is an ongoing professional relationship that helps people produce extraordinary results in their lives, careers, businesses or organizations. Through the process of coaching, clients deepen their learning, improve their performance, and enhance their quality of life."

     The Year of Momentum Program is a coaching-based cohort designed to guide women through a full-year journey of self-discovery, intentional action, and personal transformation. 

    It includes four live, two hour sessions of activities, coaching and reflection on pre-established dates found in this agreement. These sessions will be held via Zoom, or a similiar online platform if needed.

    Participants have the option to join a group channel hosted on WhatsApp to communicate with and get support from other participants and coach between live sessions.

  • Coach Responsibilities:

    1. Coach agrees to maintain the ethics and standards of behavior set by the International Coach Federation“(ICF)”. www.coachfederation.org/ethics 

     2. Coach agrees to maintain confidentiality as detailed below: 

    • This coaching relationship, as well as all information (documented or verbal) that the Paricipant shares with the Coach as part of this relationship, is bound to confidentiality by the ICF Code of Ethics but is not considered a legally confidential relationship (like in Medicine or Law). The Coach agrees not to disclose information pertaining to the Participant without the Participant's written consent.
    • Confidential information does not include information that: (a) was in the Coach’s possession prior to its being furnished by the Participant; (b) is generally known to the public or in the Particpant's industry; (c) is obtained by the Coach from a third party, without breach of any obligation to the Participant; (d) is independently developed by the Coach without use of or reference to the Participant'sconfidential information; or (e) that the Coach is required by law to disclose (ex: illegal activity,  pursuant to valid court order or subpoena; concern for imminent or risk of danger to self or to others; etc.). Where Coach reasonably believes one of the above circumstances is occurring, they may inform appropriate authorities.
    • If a Participant is under the age of 18, Coach will speak with their parent/guardian prior to beginning the coaching relationship to ensure both the parent/guardian and Participant understand the expectations of this confidentiality.
    • According to the ethics of the coaching profession, topics may be anonymously and hypothetically shared with other coaching professionals for training, supervision, mentoring, evaluation, further coach professional development and/or consultation purposes.

    Please note that this is co-hort based program; as such, information that participants share may be heard by other participants. A group norm will be set that information shared in the group should not be shared outside of the group. 

  • Program Participant Responsibilities:

    1. Participant is responsible for creating and implementing their own physical, mental and emotional well-being, decisions, choices, actions, and results. As such, the Participant agrees that the Coach is not and will not be liable for any actions or inaction, or for any direct or indirect result of any services provided by the Coach. Participant understands coaching is not therapy and does not substitute for therapy if needed, and does not prevent, cure, or treat any mental health or medical conditions.

    2. Participant understands that coaching is not to be used as a substitute for professional advice by legal, mental, medical or other qualified professionals and will seek independent professional guidance for such matters.

    3. Participant agrees to communicate honestly, be open to feedback and assistance and create the time and energy to participate fully in program.

    4. Participant understands that the dates established for the group sessions are set ahead of time and will not be moved, barring extunating circumstances. Sessions will not be recorded. As such the participant is responsible for attending sessions. Dates for sessions include:

    Four Live Virtual Group Sessions on Zoom: (9-11 am PST)- 8 hours total 

    • Saturday, January 3, 2026
    • Saturday, May 2, 2026
    • Saturday, September 12, 2026
    • Saturday, December 5, 2026

    5. Participant acknowledges that all activities and reflection worksheets used in teh program are the property of Momentum Coaching & Consulting, LLC, and cannot be duplicated without express written permission. 

  • Payment Options

    Please select the option that is right for you below. Once this agreement is received, you will receive an invoice via the payment platform Wave (WaveApps) from Momentum Coaching and Consulting. Participants must be current on their payments to particpate in any session. 

  • Release of Information:

    The Coach engages in training and continuing education pursuing and/or maintaining ICF (International Coach Federation) Credentials. That process requires the names and contact information of all Clients/Participants for possible verification by the ICF. By signing this agreement, you agree to have only your name, contact information and start and end dates of coaching shared with ICF staff members and/or other parties involved in this process for the sole purpose of verifying the coaching relationship; no personal notes will be shared.

  • Limited Liability:

    Except as expressly provided in this agreement, the Coach makes no guarantees or warranties, express or implied. In no event will the Coach be liable to the Participant for consequential or special damages.

    Notwithstanding any damages that the Participant may incur, the Coach’s entire liability under this agreement, and the Participant's exclusive remedy will be limited to the amount paid by the Participant to the Coach under this agreement for all services rendered up until the termination date.

    This is the entire agreement between the parties and reflects a complete understanding of the parties with respect to the subject matter. This agreement supersedes all prior written and oral representations. If a dispute arises out of this agreement that cannot be resolved by mutual consent, the Particpant and Coach agree to attempt to mediate in good faith for up to 30 days after notice given. If the dispute is not resolved, and in the event of legal action, the prevailing party shall be entitled to recover attorney’s fees and court costs from the other party.
