ELEVATE WEBSITES DESIGN — ONBOARDING QUESTIONNAIRE

Purpose: This form gathers everything we need to build your website’s design, copy, and SEO. Please answer as completely as possible. Your responses will directly shape the tone, visuals, and structure of your new site.
SECTION 1 · BUSINESS OVERVIEW & BRAND FOUNDATION

Used for homepage messaging, About page, and positioning
1. Business name (as it should appear on the website)*
2. Short tagline or slogan (if any)

SECTION 2 · IDEAL CLIENT & MARKET POSITIONING

Used for tone of voice, SEO targeting, and copy direction

SECTION 3 · SERVICES & TREATMENTS

Used to build your Service Pages — sitemap already handled internally
1. Please list and describe all your services or treatment's. For each, include: Service name, Short description (what it is or what it does), Duration or session length, Price (if you’d like it displayed)
2. Should any services be grouped into categories on the site?
Example: “Spa Treatments,” “Body Therapies,” “Add-Ons”

SECTION 4 · INTEGRATION ACCOUNTS (CALENDLY & STRIPE)

Only confirmation questions — automation already handled
1. Please list each of your services (e.g., Massage, Facial, Moroccan Bath, etc.) and indicate which ones should be bookable independently; meaning a client could schedule that service even if another appointment is already booked at the same time.
Example: “Facials and Massages should each be bookable at the same time, but Moroccan Bath should block other bookings.”
2. Do you already have separate Gmail accounts to connect with each service’s calendar?
3. Do you already have a Calendly account? If yes, provide the email used or your booking link
4. Do you already have a Stripe account? If yes, provide the email used or your Stripe dashboard link.

SECTION 5 · DESIGN STYLE & VISUAL IDENTITY

Used for mood board, color palette, and photography plan
1. Do you have preferred brand colors or an existing palette?
2. Describe your preferred design style (modern, minimal, soft, luxurious, etc.).
3. List up to 3 websites you like and explain what you like about them.
4. Are there any design styles or elements you dislike?
5. Do you already have professional photos or videos we can use? (Yes/No — upload files to Google drive if yes)

SECTION 9 · FILE UPLOADS & ASSET DELIVERY

All files will be collected via Google Drive. not through this form.
Please upload all visual assets using the shared Google Drive folder provided to you after completing this form.

Folder name: AlemSpa – Onboarding Assets

Inside the folder, include sub-folders for:

• Logo Files: SVG format preferred (vector). If SVG isn’t available, PNG is fine — it may load slightly slower.
• Team Headshots (only if including a Meet the Team section)
• Business Photos or Videos (interior, products, treatments, etc.)
• Testimonials or Written Materials (optional)
• Consent Form: Upload your current consent form (PDF or Word) under a subfolder named “Legal Documents.”

Guidelines:

• Clearly name each file (e.g. logo_primary.svg, team_alem.jpg, treatment_room1.png).
• Please ensure the images are high-resolution and properly cropped before upload.
• If you experience any issues, simply share the folder link with Elevate Websites Design via your email thread.

This approach keeps your files secure, avoids Jotform storage limits, and lets us organize your visuals efficiently for design and SEO use.
