Play Date Crèche Waiver

ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT

In consideration of being allowed to participate in Stay & Play sessions, birthday parties, and other activities provided at Play Date Kids (collectively, the “Services”), the undersigned parent or guardian (“I” or “Parent/Guardian”) acknowledges and agrees to the following terms:

1. Acknowledgment of Risks
I understand and agree that Play Date Kids is a children’s play space, and participation in play activities (including but not limited to the use of play equipment, toys, soft play, jumping castles, and other recreational facilities) carries inherent risks. These may include falls, collisions, exposure to illness, and other incidents that may cause injury or illness to my child.

I acknowledge that the risks described above are not exhaustive and that other unanticipated risks may also result in illness, injury, or property loss.

2. Parent/Guardian Responsibility
I understand that I am required to remain on the premises at all times while my child is participating in Play Date Kids activities.

While Play Date Kids staff and carers are present to provide supervision of the environment, I accept that ultimate responsibility for the care, safety, and wellbeing of my child rests with me.

I accept full responsibility for any injury, illness, or incident involving my child during their participation, whether arising from use of equipment, interactions with other children, or otherwise.

I acknowledge that Play Date Kids cannot be held liable for any illness or injury sustained by my child.

3. Respectful Play
I agree to ensure that my child:

Uses the play facilities in a safe and responsible manner.

Treats all other children and families in the space with respect, regardless of age.

Follows any instructions provided by Play Date Kids staff to ensure the safety and wellbeing of all participants.

4. Release of Liability
In consideration of being permitted to access Play Date Kids, I, on behalf of myself, my child, and our heirs, hereby voluntarily release, discharge, and hold harmless Play Date Kids, its owners, staff, contractors, and volunteers from any and all liability, claims, demands, or causes of action relating to injury, illness, loss, or damage that may arise from participation, except where required by law.

5. Indemnification
I agree to indemnify and hold Play Date Kids harmless from any claims, costs, or liabilities brought by a third party as a result of any act, omission, or misconduct by me or my child while participating in Play Date Kids activities.

6. Media Consent
Unless I provide written notice otherwise, I grant Play Date Kids permission to use photographs and/or video recordings of me and/or my child taken during sessions for promotional, educational, or marketing purposes, without compensation.

7. Acknowledgment
By signing below, I confirm that:

I have read and understood these Terms & Conditions.

I am the parent or legal guardian of the child named in the registration.

I am over 18 years of age and competent to contract in my own name.

I voluntarily accept full responsibility for my child’s participation at Play Date Kids.
