    Youth Basketball Team Registration & Roster Form
  • TEAM INFORMATION:

    One registration form PER team!

    Once a team registration & roster has been received, you will get a link for the liability waiver, conduct and league rules for ALL players & parents to complete.

    NO players or coaches will be allowed to participate unless these forms have been completed.

    PLAYER ELIGIBILITY:

    • 2025-2026 GRADE LEVEL USED FOR REGISTRATION

    • All players will be required to have a valid AAU Card for this season. AAU ID will be requested when completing individual athlete registration form. https://aausports.org/membership

  • *Please note that while we only ask for the players information in the above roster field, you will be required to show proof of Grade or Date of Birth if there is ever a player in question. You should have this information readily available at all games and practices.*

  • AAU MEMBERSHIP AND SEASON GUIDELINES

    Players/Guardians accept and understand that in order to participate as a coach and/or player, they are required to have a valid AAU Membership per our insurance requirements. 

    Coaches need to submit a roster of their team a week prior to start of season. Coach AAU numbers and all player AAU numbers must be listed on the roster a week prior to the start of the season. Final roster with changes and jersey numbers are due 2 days prior to the start of the season. 

    AAU Numbers will be checked a week before the start of the season. Any coaches or players not in compliance will be noted. They will have until 5pm of the Thursday before the start of the league season to send correct/updated AAU Membership numbers. 

  • Coaches Code of Conduct for MVL Basketball League


    To ensure a positive and respectful environment, all coaches are expected to adhere to the following Code of Conduct:


    1. Respect and Sportsmanship

    • Treat all players, parents, officials, and opposing teams with respect and dignity.
    • Refrain from using abusive, offensive, or derogatory language or behavior.
    • Promote good sportsmanship by modeling fairness, humility in victory, and grace in defeat.

    2. Safety and Well-being

    • Ensure a safe environment by following all safety protocols and addressing hazards promptly.
    • Be attentive to players’ health, including hydration, rest, and injury prevention.

    3. Integrity and Fair Play

    • Abide by the rules and regulations of the league and the game.
    • Respect the decisions of referees and officials, even in disagreement.

    4. Positive Communication

    • Communicate constructively with players, parents, and league officials.
    • Provide feedback that is encouraging, specific, and focused on improvement.

    5. Role Model Behavior

    • Lead by example, demonstrating honesty, patience, and respect at all times.
    • The league has a zero tolerance policy for profanity, inappropriate behavior and offensive language or gestures.

     

  • By signing below, I confirm that all information provided is true and accurate. Knowingly submitting false information will result in disqualification from the Maui Versatility Basketball League.

    By signing below, I also agree to uphold the Coaches Code of Conduct and understand that failure to comply may result in disciplinary action, including suspension or removal from the league.

    You also acknowledge payment and registration must be received five days before the first game.

    Once coaches fill out the Team Registration, they will be given the individual player registration link for players to register and pay.
