1. A NON REFUNDABLE Deposit of R200. is payable with this application.
2. One term's notice in writing must be given before any child is removed from the School, failing this a full term's fees will be charged in lieu of notice.
3. Fees and levies are payable in advance.
4. In the event of any information given by the Parent or Legal Guardian on this form proving to be incorrect, the Committee is entitled to refuse to allow the child to continue attending the School and all fees already due will be payable. No refunds will be given for money paid in advance.
5. Any change of address or phone number at home or at work must be notified to the Secretary. This applies before and after admission to the school.
6. I hereby declare that the information I have given is true and correct in every respect and that I have read and understood these forms. I agree to be bound by the Rules and Regulations as above and I understand that completion and submission of this form does not necessarily mean that my child will be admitted to the school.
7. I/wedo hereby specifically, irrevocably and indefinitely indemnify and hold blameless the Chabad of Sandton nursery school against any loss, damage or injury which may be sustained by my child at school or en route thereof, or in the course of any extra-mural activity, or and school outing in which he/she may participate. Also, I/we hereby give consent for the school to arrange any medical assistance, when necessary, as the school sees fit.
8. The Committee's decision on all maters relating to the School and admission thereto is final.
