    Thank you for submitting a nomination package for consideration of induction of your candidate into the Adaptive Golf Hall of Fame. Scoring is objective and candidates are placed in an Order of Merit based on that scoring and subsequent Selection Committee discussion and evaluation. A majority of Selection Committee members is required to establish the Order of Merit (rankings) upon which, pending the number of candidates to be inducted in a given year, voting will take place. To be inducted, a candidate must receive 75% of the votes from the entire Selection Committee.

    • Nominations remain confidential to the Selection Committee. • The candidate nominated will not be notified by the National Alliance or any Selection Committee Member their name has been submitted as a candidate, or by whom, for induction into the Adaptive Golf Hall of Fame. • Selection Committee Members may not nominate themselves for induction into the Hall of Fame, but may nominate others. • The Selection Committee will notify the nominators if their candidate was, or was not selected. • If selected, the Committee will coordinate with the nominator as to notifying the Hall of Fame inductee. • Any candidate not selected for induction shall remain in consideration for two (2) more years without requiring the resubmission of a nomination form.

If the inductee is deceased, the nominator will facilitate notification to the inductees representative (family member or associate).

  • Candidate Justifications

    Please note the word count maximums for each area when pasting content in from another document. These maximums must be observed. If the candidate is/was primarily a golfer themselves, incorporate playing experience/record, major tournament victories, etc. Consideration for the Hall of Fame goes beyond playing to include the use of their skills in advocacy efforts and growth of the game initiatives through playing, service and leadership.
  • Service and Leadership to the Game

    Service and Leadership for a sustained period of time is a critical aspect of a Hall of Fame career. Please describe the candidate’s biography and overall service and commitment to the adaptive golf community over an extended period of time and/or influence at a level that exceeds others.

  • Growth of Game Impact

    Efforts over time that have facilitated entry into the game for individuals with disabilities is a critical aspect of a Hall of Fame career. Packages should statistically quantify the candidate’s impact in the game in terms of their personal responsibility in the growth of programs, new golfers brought to the game, coaches trained and educated, or more.

    A Hall of Fame candidate should have been instrumental, if not responsible for the creation of initiatives individually and/or with others that directly led to, or continue to lead to individuals with disabilities being able to participate in the game of golf. Please identify these initiatives and their byproducts.

  • Previous Awards and Honors

    Nomination packages should identify any previous golf industry awards or awards received from other industries during the candidate’s career related to their work in support of individuals with disabilities. Previous awards indicate a pattern of performance over time. Award quality is being evaluated, not the quantity of awards received.

    Nomination packages must include one endorsement, but no more than two (2 max) from individuals related to the candidate to augment the nomination. Maximum of 500 words per endorsement. 15 points maximum are available for endorsements, whether one or two are submitted.

    As your FINAL STEP, please upload the following to help "paint the picture" of your candidate "in action" to further assist the Selection Committee in their evaluation process. 

    • Images: Up to three (3) additional photos of the candidate in action with others during their career.  
    • Video (optional but encouraged): Up to one (1) short video, less than two minutes that could further inform the selection committee
