CHECK-IN AGREEMENTS
1. I verify that our team roster will be updated and uploaded to the tournament registration portal. This roster will contain all players who will participate in the event. I understand I may not update this roster after our first game has kicked off. I understand this roster will be checked prior to the event by the tournament committee.*

YES I UNDERSTAND OUR TEAM ROSTER RESPONSIBILITY TO UPLOAD an UPDATED ROSTER and THAT NO CHANGES TO THE ROSTER MAY OCURR AFTER OUR FIRST GAME KICKS OFF.
2. I verify that every player listed on the roster is of the correct age for the age division they are participating in this event. Each player's listed birthdate on the team roster is correct. Only players listed on the team roster will participate. I verify each player participating is registered in the upcoming current seasonal year, is of good standing, and is insured.*

YES I CAN CERTIFY AGE OF EACH PLAYER, THAT EACH PLAYER LISTED IS REGISTERED, INSURED, IS A PLAYER IN GOOD STANDING & ONLY THOSE LISTED WILL PARTICIPATE
3. I verify that our team has in possession a proof of birth for every player participating in this year's event. That proof of birth is either in the form of birth certificate, ID Card, Driver's License, or current/past player pass.*

4. I verify that our team has in possession a medical release/waiver document (either USYS or US Club). We will keep these on hand at all times during our team's participation so that if needed they can be produced at the fields.*

5. I UNDERSTAND THAT THE TOURNAMENT EVENT DOES NOT OFFER INSURANCE ADDITIONAL TO THE INSURANCE PROVIDED BY OUR PLAYER'S REGISTRATION AND THAT WE ARE RESPONSIBLE FOR SECURING THIS INSURANCE VIA PROPER PLAYER REGISTRATION VIA OUR HOME CLUB'S REGISTRAR*
