Awards Form 2026

Thank you for nominating someone for NBDF's Awards of Excellence! We're looking forward to learning more about your nominee. Before you get started, here's a few things you should know:

1. Try to answer the questions as fully as you can. The more information we have, the better we will be able to consider the nomination.

2. Please don't nominate yourself. Any self-nominations will not be accepted.

3. You can submit the same person for more than one award, but you will need to fill out a new form for each submission.

4. If you have questions about the award criteria, you can refer to our Bleeding Disorders Conference website. (link)

5. If you have questions about the process, you can email communications@bleeding.org and we will do our best to answer them.

6. If the person who you're nominating has been selected for the award, you will both be notified in July 2026.

7. The form must be fully filled out and submitted to be considered. You can save your progress and return to it later, but we do recommend having everything ready before filling out the form. It can be helpful if you review the form first, so you can gather any information you might need before filling it out.

Tip: after reviewing the questions, you can type the answers out in a Word document and cut and paste them into the answer box.

Please note: many of these fields are required, meaning you must answer them in order to submit the form. However, some questions won't be applicable to your particular award. In those instances, you can just put "N/A."

We're excited to learn more about your nominee. We assess the nominees based on five criteria:
-their IMPACT on the bleeding disorders community,
-their INNOVATION and LEADERSHIP
-how well they ADVOCATE or EDUCATE to or for the bleeding disorders community
-their DEDICATION and COMMITMENT
-their COLLABORATION or COMMUNITY ENGAGEMENT.
