NTX Soccer A&D Report Submission

Before submitting this form, please ensure you have gathered all required documentation to avoid delays in the review process. Each Referee Misconduct Report must include the Game Day Report, the Referee Supplemental Report (available at all fields on game day), and written statements from the referees assigned to the match. These reports should be submitted within 48 to 72 hours of the incident. Submissions missing any of the required documents will be considered incomplete and may be returned for additional information.

Each Referee Misconduct Report submission must include the following items:

• Game Day Report
• Referee Supplemental Report (available at all fields on game day)
• Statements from all referees assigned to the game
