 I understand that employment with Essential Hands Home Healthcare, LLC is contingent upon the successful completion of all pre-employment screenings, including a criminal background check in accordance with Ohio state law and the Ohio Department of Aging’s requirements for home care providers.

I hereby authorize the Company and its authorized agents to obtain and review my criminal history, driving record (if applicable), and any other background information relevant to the position applied for. I understand that a criminal conviction will not automatically disqualify me from employment but will be considered in relation to the position and in compliance with applicable state and federal laws.

 4. Drug Testing and Health Requirements
