• This form is an appointment request, not a guaranteed booking.

• Once I review your idea, I’ll contact you with pricing, available dates, and any questions.

• A non-refundable deposit is required to secure your appointment.

• No deposit = no appointment.

• The deposit goes toward the final price of your tattoo.

• If you need to reschedule, please let me know at least 48 hours in advance so your deposit can be transferred to a new date one time.

• Same-day cancellations, no-shows, or arriving more than 15 minutes late may result in losing your deposit.

• You must be 18+ with a valid ID on the day of your appointment.

• I reserve the right to decline any design or request that doesn’t align with my style or values.
