Acknowledgment and Agreement I hereby agree and accept the terms and conditions of allowing my child(ren) to participate and remain inside the premises of Isaiah 58 Skate House. I understand and acknowledge that by permitting my child(ren) to be inside the rink, I assume full responsibility for their conduct, safety, and well-being, and release Isaiah 58 Skate House, its owners, staff, and affiliates from any liability arising from injury, loss, or incident occurring on the premises.

Isaiah 58 Skate House Comprehensive Liability Waiver, Assumption of Risk & Indemnification Agreement** In consideration of being permitted to enter, use, or participate in any capacity at the facilities, premises, programs, or events operated by Isaiah 58 Skate House (“the Company”), I, the undersigned, for myself, my heirs, executors, administrators, legal representatives, successors, and assigns (and for any minor for whom I am parent or legal guardian), hereby release, waive, discharge, and covenant not to sue the Company and its owners, officers, directors, members, managers, employees, agents, volunteers, insurers, affiliates, successors, and assigns (“Released Parties”) from any and all claims, demands, causes of action, damages, losses, liabilities, judgments, costs, or expenses of whatever kind or nature—whether in law or equity, known or unknown, foreseen or unforeseen—arising out of or relating to my access to or participation in roller-skating, recreational, instructional, or social activities at or sponsored by the Company. I understand and expressly acknowledge that such activities involve inherent and unpredictable risks, including but not limited to falls, collisions with objects or persons, equipment malfunction, surface defects, erratic conduct of others, and the possibility of severe bodily injury, permanent disability, or death. I knowingly and voluntarily assume all risks, both inherent and incidental, whether caused by the negligence of the Released Parties or otherwise, and accept full responsibility for any resulting injury, property damage, or loss. I further agree to defend, indemnify, and hold harmless the Released Parties from and against any and all claims, suits, actions, damages, or costs (including reasonable attorneys’ fees and court costs) arising from or related to my participation or conduct on the premises. I acknowledge that the Company provides no express or implied warranties regarding the safety, maintenance, or suitability of the facility or any equipment and that I am solely responsible for determining my own ability and safety equipment needs. I further agree that this Agreement shall be governed by and construed under the laws of the State of Maryland, that any legal action shall be brought exclusively in a court of competent jurisdiction in that state, and that if any provision herein is held invalid, the remainder shall continue in full force and effect. By signing below, I affirm that I have carefully read, fully understand, and voluntarily agree to the terms of this Waiver and Release, intending to be legally bound to the fullest extent permitted by law.
