    Each business or nonprofit may apply for only one project under this grant cycle.

    If an applicant encounters an error during submission or needs to update information, they may resubmit their application using the same link. The most recently dated submission will be considered the official version for review.

    Please note that the Economic Development Department cannot make corrections or edits to individual applications once they have been submitted; applicants are responsible for reviewing all information carefully prior to final submission.

    The following questions help determine your qualification for this grant. Some questions may not apply to all businesses. If a question is not relevant to your business, simply mark 'Not Applicable" (N/A)
  • NAICS, or the North American Industry Classification System, is a way to classify businesses using a six-digit code. To identify the proper code for your company, you can use the Utah Reference Guide or https://www.census.gov/naics/ to self-identify the code that best reflects your primary business activity (revenue producing activity).

  • Scope of Work - General Project Information

    Describe your proposed idea or project. We will do X activities with the funds. What will go into getting the project done? Are there any partnerships that will support the project?
  • Deliverables and Performance Measures

    Describe what your project will accomplish once completed.What specific results do you expect? Think about how this project will help your business grow, reach more customers, improve services, or create new opportunities. Examples: 1. Buying a bread slicer will let us sell pre-sliced loaves to seniors and increase weekly sales by 20 loaves. 2. We will purchase new lab equipment (deliverable), allowing us to expand local testing services and reduce the need for residents or businesses to send samples out of the county (performance measure). 3.Adding outdoor seating will attract more customers and increase foot traffic downtown.Be as specific as possible about the results you expect. These details help us understand the value and progress of your project.
  • Anticipated Economic and Community Impact

    Describe how your project will help your business grow and benefit San Juan County’s economy. You may want to include ways the project could create or retain jobs (including self-employment), increase business revenue, sales tax, or transient room tax (TRT), or help your business operate more efficiently. You can also share whether the project expands or accelerates your work, improves facilities or equipment for long-term sustainability, or strengthens the local economy and community overall. // For programs, events, or community initiatives: Describe how your project will support workforce development, education, or community learning opportunities that strengthen San Juan County’s long-term economic vitality. Examples may include training programs, workshops, youth or adult skill-building, business resource events, or initiatives that connect residents and employers. You may also include how your project will increase local participation, partnerships, or access to information and resources.
  • Project Schedule and Action Plan

    Provide a breakdown of how your project will be completed, identify the tasks that need to be completed, then outline the sequence of activities that will follow. How will you show progress toward completing the project? Make sure your plan outlines a clear and realistic path to completion within 10-12 months while ensuring that deadlines are achievable, and your project stays on schedule. (Applicants who do not complete the grant requirements will be ineligible for future grants.)
  • Budget

    Provide a detailed budget, including your required matching funds. The amount of funding you request will determine the required matching funds. Your match must be calculated as a percentage of the total grant request, based on the tier you select. Matching funds can be provided in cash or in-kind contributions (such as labor, materials, or equipment use).
  • Please download and use the official Excel Budget Template provided below. Budgets submitted in any other format will not be accepted.

    Complete all sections of the template before uploading your file. Make sure your budget clearly shows how your total project cost equals your grant request plus your match amount (cash and/or in-kind).

    In the budget template, explain the funding requirements of your project, including any bids, quotes, or estimates used to calculate your costs. If applicable, include an estimated cost to administer the grant (such as staff time or overhead).

    Important: Any match amounts (cash or in-kind) listed in your budget must be supported with uploaded documentation in the appropriate match upload section.

    The amounts shown in your budget template and supporting match documentation must match exactly. Applications with unmatched or undocumented match amounts will be considered incomplete.

  • Budget Narrative

    Provide a Budget Narrative to justify your budget expenses.Explain what each item is, why it is needed, and how it will be used in your project. Include how the cost was determined (e.g., vendor quotes, industry standards, or past expenses).This section helps the County understand how you plan to use the funds and supports award documentation. Be sure to include the amount, description, and purpose of each expense.Also describe how you will meet your match requirement. Will you do it through cash investment, other grants, in-kind labor or materials, or other verified sources.Your Budget Narrative must match your Excel Budget Template and supporting documentation.
  • Supporting Documents

  • The following documents are required for your application to be complete.

    • Budget 
    • W-9 (completed and signed) This form will be used to process your award payment if selected. Please make sure it is signed and matches your IRS tax records exactly, including your legal business name, tax ID, and mailing address.
    • Valid Business License (San Juan County, a local municipality, or Navajo Nation)
    • For nonprofits: IRS Determination Letter
    • Tier 3 applicants must submit a Profit & Loss Statement (prior and current year)
    • Proof of Match: either cash or in-kind. If you are providing both, upload them separately. Documentation of matching funds can be provided in various forms. You can always contribute more than the required match — but not less. Below are suggestions:
      • Cash Match:
        • Bank statements showing available funds
        • A letter from an investor or lender confirming financial commitment Proof of personal or business funds designated for the project (e.g., financial statements)
        • Grant award letters from other funding sources
      • In-Kind Match: (non-cash contributions that have a measurable fair market value.)
        • Donated labor or volunteer time - A letter stating the estimated value of the labor based on industry wages
        • Donated materials or equipment use - A letter from the donor specifying the contribution and its estimated value
        • Discounted services - An agreement from a contractor or service provider detailing the discount provided
        • Existing business assets used for the project - Proof of ownership anda valuation estimate

    Applicants who provide the following optional supporting documentation will receive additional points in the evaluation process, though submission is not mandatory. These optional uploads will better prepare applicants for state-level programs such as the Utah Economic Assistance Grant (EAG):

    • Profit and Loss Summary, Balance Sheet, or Statement of Activities (for nonprofits)
    • Letter(s) of Support from a community representative or an individual who will directly benefit from this project, highlighting the anticipated impact and significance of the initiative on local businesses, residents, or organizations.
    • Vendor quotes or cost estimates for project or equipment purchases.

    By accepting grant funds through this program, I acknowledge and agree to the following terms:
  • Privacy Statement

    This application is processed through JotForm, a secure third-party platform, ensuring that your personal information stays protected. JotForm encrypts all submissions using 256-bit SSL encryption (the same level used by banks) and meets top security standards and is PCI DSS compliance.

    Once the application period closes, we will securely store necessary data on county servers for record-keeping purposes. Any access to information on JotForm will be discontinued as part of our standard process.

    San Juan County does not share your personal data with outside entities. Your information is used only for the purpose of processing this application—nothing more.

    Verification & Compliance

    I authorize the San Juan County CED Board to verify the information provided in this application and accompanying documentation. I agree to provide any necessary information or records upon request to confirm compliance with the grant requirements. If awarded, I understand I will receive a 1099 from San Juan County for funding received and agree to retain documentation related to this application and any use of grant funds for at least five (5) years. 

    Funding Disbursement & Reporting

    If awarded, I understand that 40% of the funds will be provided upfront, with the remaining 60% reimbursed upon submission of project progress reports, including receipts. Failure to submit the required documentation may result in delayed or withheld reimbursement. Applicants requesting funding for equipment purchases must provide a valid sales quote or vendor invoice detailing the cost before receiving any grant funds for that expense.


    Project Completion and Compliance Requirement

    I understand that if my business fails to complete the approved project, misuses funds, or does not comply with reporting requirements, I may be required to repay funds received and may be ineligible for future grant opportunities through the San Juan County CED Board.

Limited funds are available, and meeting eligibility requirements does not ensure approval. Applicants are encouraged to submit complete and well-documented proposals to maximize their chances of selection.
