Liability Release Agreement
ACKNOWLEDGMENT AND CONSENT I, an adult or the undersigned parent or legal guardian of the above-named student, hereby give permission for my child to travel by bus to and from the specified sporting event under the supervision of school staff and/or authorized volunteers. I understand that reasonable precautions will be taken to ensure the safety and well-being of all participants. However, I acknowledge that participation in school trips involves inherent risks, including but not limited to transportation-related incidents, personal injury, or property loss. RELEASE OF LIABILITY In consideration of my child’s or my participation in this trip, I hereby release, discharge, and hold harmless the school, its employees, volunteers, and agents from any and all claims, demands, actions, or causes of action arising out of or in connection with my child’s or my participation in this trip, including transportation to and from the event, except for acts of gross negligence or willful misconduct. TRANSPORTATION AGREEMENT I understand that all students are required to follow school rules and the directions of the supervising staff while on the bus and during the event. Failure to do so may result in disciplinary action or loss of participation privileges in future trips.
