Have A Heart TNR is offering $25 per cat spay - neuter with rabies vaccine included. Cats that cannot be handled safely by vet staff must be in traps.
Carefully read and understand all the following before you sign your name.
1. Cat or cats must be friendly and safely handled by vet staff. If not, they must be in traps.
2. I understand that there is some risk of injury or death in the procedure with the use of anesthetics and drugs.
3. I understand if the cat is pregnant at the time of surgery, the pregnacy will be terminated.
4. The surgery will be performed by Tallgrass Veterinary Clinic, 1457 Union Rd, Concordia KS. 785-243-7400
5. I understand that my cost is $25 per cat, which is to be paid to a representative of Have A Heart TNR before a voucher is given. After you have purchased the voucher or vouchers, it is your responsibility to call Tallgrass and schedule the appointment. It is also your responsibility to transfer the cats to Tallgrass and pick up the cats after surgery. Drop off is normally 8 am and pick up is 4 pm.
6. When you call to make the appointment, tell the receptionist that you have a $25 low cost voucher from Have A Heart TNR and how many cats you need done. When you drop off the cat or cats for surgery, you must give the voucher or vouchers to the receptionist at Tallgrass Veterinary Clinic.
7. Each voucher will cover the cost of spay - neuter surgery and rabies vaccine for one cat. If you want any other vaccines or procedures done, that will be your responsibility and you will be billed by Tallgrass.
8. *** IMPORTANT *** You must have a voucher or vouchers from Have A Heart TNR before you call to make an appointment. If you don't have a voucher or vouchers, don't schedule an appointment unless you want to be charged full price.
*** I willingly assume the risk and responsibility of participating in the program. I hereby release Have A Heart TNR inc., all veteriniarians and assistants from any and all claims arising out of or connected with the performance of this program and procedure or any adverse reactions to vaccinations. I agree that I will not claim any right of compensation from them, or file action by reason of such sterilizations or any consequences related thereto.
