By checking the box in the Permission Agreement below, and by affixing my signature or mark in the signature box, I hereby give permission for my child (or for myself if over 18), to participate in the Jazz Arts Academy program. I also acknowledge that I have read the above terms and conditions specifically realated to the photo release and the policies and procedures statements.
I also understand that if my child displays any disruptive or disrespectful language or behavior to the staff or to other students in the program that I will be notified immediately by telephone. If the behavior is not corrected at once my child will no longer be able to participate in the program.
