Please fill out the information on the appropriate form based on the age of individuals for each immediate family member in your party. Only parents/guardians can sign for minors. Each adult participant should sign their own form. Signatures on this form are electronic and may be signed with a mouse or your finger on a tablet/phone.

If you are bringing a child that is not part of your immediate family, please give the following link to the child's parents and have them fill out the form and purchase their wristbands. Online Unaccompanied Minor Wristband and Permission Form.
