    Technology/Image Consent Form

  • If a student is under the age of 18 when they begin attending Mid Michigan College, a parent/legal guardian must review and consent to the following releases. This consent remains valid until the student reaches the age of 18 or until revoked in writing (such revocation to be submitted to dual@midmich.edu) by the parent/legal guardian, whichever occurs first.

  • Technology & Internet Access Release

    Mid Michigan College provides students with access to a wide variety of technology resources, including internet access, online learning platforms, and third-party tools and services. These tools are essential for completing coursework and participating fully in college-level courses. The College does not endorse or warrant the security, privacy practices, or content of third-party services.

    Because your student is under 18, some online services may be restricted by default. By signing this form, you acknowledge and authorize the following.

    Access to Online Tools and Services

    • I give permission for my student to access digital resources required for their courses, which may include services such as YouTube, Google Workspace, Respondus, and other learning technologies.
    • I understand that these services are not specifically designed for minors and may contain unfiltered content.

    Internet Access and Content

    • I understand that my student will have access to the open internet and that Mid Michigan College does not restrict content beyond what is required by law or College policy.
    • I acknowledge that I have been provided access to the College's Acceptable Use Policy and have had the opportunity to review it.
    • I acknowledge that while faculty and staff strive to create safe and professional learning environments, the College cannot guarantee that all materials encountered will be suitable for younger audiences.

    Privacy and Data Sharing

    • I understand that certain online tools may collect personal information about my student, which may include name, email address, student ID number, coursework, and usage data.
    • I understand that the College's use of my student's information is governed by the Family Educational Rights and Privacy Act (FERPA), but that third-party services may have their own privacy policies that are not controlled by the College.
    • I acknowledge that my student’s educational records are protected by FERPA, and that as the parent/legal guardian of a minor student, I retain rights under FERPA to access my student's educational records at the high school. I understand that these rights transfer to my student when they turn 18 or when they enroll in a postsecondary institution. Therefore, I understand that I will not have access to my student's educational records at Mid Michigan College without the student's written consent.

    Responsibility for Conduct

    • I understand my student is expected to follow all College policies regarding appropriate technology use and academic integrity.
    • Misuse of College technology resources may result in disciplinary action by the College in accordance with the College's Student Code of Conduct and/or the student’s high school if the student is dual/concurrently enrolled. Parents/legal guardians will be notified in writing within five (5) business days of any disciplinary action taken against minor students, except where prohibited by law or where the student is an eligible student under FERPA.

    By signing below, I consent to my student’s use of College technology resources and access to the internet as part of their courses.

  • Course Specific Image Release

    I understand that courses may be recorded for use in future semesters when this course is offered again, for faculty development, and in the current semester for students in different sections of the same course. I understand that these recordings may be retained by the College for up to 5 years after completion of the course. For these purposes, I permit Mid Michigan College to use and, when necessary for educational purposes within the College or to other educational institutions for legitimate educational purposes, release my student's education records that contain the voice or likeness of my student when they are participating in courses, including asking questions, participating in a discussion or group work, and making presentations.

    I authorize the release of work that my student has created in courses (e.g.,
    PowerPoint slides when making a presentation, a video created for course-specific social media channels, etc.) for educational purposes within the College. I understand that, the instructor will redact my student's name and any other personally identifiable information from the assignment before any use beyond the current course section, and that the instructor is prohibited from releasing grades associated with my student's work. I understand that if the College intends to use my student's work outside the College or on public platforms, I, or my student if they have reached the age of 18 or become an eligible student under FERPA, will be contacted to confirm my/their consent to this release and I/my student will not receive compensation.

    I understand that my consent to the Course Specific Image Release is voluntary and not a condition or requirement for my student's participation in the course or enrollment at Mid Michigan College.

  • Multimedia Release

    I hereby agree that the Board of Trustees of Mid Michigan College (the “College”), and those acting pursuant to its authority, shall have the right to record my/my student's participation, appearance, likeness and voice on videotape, audiotape, photograph, digital, electronic, or in any other medium and I hereby unconditionally and irrevocably consent to the College’s use of such materials for any legal purpose the College deems appropriate for the benefit of the College, including commercial and advertising purposes.

    I also, hereby forever waive (i) any right to require payment or compensation from the College for the use of these materials by it or those acting pursuant to the College’s authority and (ii) the right to object to the use of such materials for any purpose permitted by this release, including, without limitation, the license or sale of such materials by the College and those acting pursuant to its authority who publish, print, display, exhibit, distribute, or otherwise publicly use any such materials for any legal purposes.

    I understand that this release grants the College the right to edit, crop, retouch, or otherwise reasonably alter such materials, at its discretion, and, at its option, to reveal my/my student's name and identity in such materials or by descriptive text or commentary. I understand and agree that any intellectual property rights associated with such materials are the sole property of the College. Furthermore, I agree that this release shall be construed in accordance with the laws of the State of Michigan and if any term or provision of this release shall be held illegal, unenforceable, or in conflict with any law governing this release, the validity or enforceability of the remaining portions shall not in any way be affected or impaired thereby.
