Change Level Request Form

This request is to change your current "benchmark" rating either lower or higher. Your "benchmark" is your designated division/level of play & you cannot play below it.
Requests should be made by the player. It is strongly suggested that you discuss this with your club pro or team captain as he/she will be contacted as part of the research process. If NO pro at your club/facility, you may be asked to be evaluated by one here in our area before a decision is made.

NO REQUESTS ACCEPTED AFTER THE PLAYER HAS PLAYED IN A CURRENT SEASON MATCH.
