    Thank you for supporting Holiday Hope.To ensure your charitable tax receipt is issued correctly, please follow the steps below.
  • How to Use This Form: You can submit your information in three ways:

    Individual

    • You personally purchased items and have the receipts. Complete your information below to receive a tax receipt.

    Group (Primary Contact Submits)

    • Your group pooled money and one person (the shopper) bought the items.
      Only the shopper should complete this form and upload all receipts, then list the names and contribution amounts for everyone who gave cash so each donor can receive a donation tax receipt.

    Group (Contributors Self-Submit)

    • Multiple people in your group made their own purchases and have their own receipts. Each contributor completes this form individually using the same Appointment ID so we can link everyone together.

    If you only contributed cash and did not shop, you do not need to complete this form. Your shopper will include your information.

    If you are unsure, choose the option that best reflects how you donated, not how the group is structured.
