By registering my athlete, I am authorizing participation in all and any activities prepared by Game Time Playz (Alec Martinez), D'Vontay Friga (Athlete), gym location, and/or sponsors during the basketball camp. I also hereby approve my athlete to be photographed, videotaped, and posted on social media for camp content or any other social media content. I also hereby understand that anyone named above, and anyone else participating in the camp, are not responsible for any injuries, lost or stolen items during this event. In case of injury to athlete, I hereby waive all claims against said organizations mentioned above, including all coaches, partners, sponsors, or participants. Some of these include, but are not limited to, the risk of injuries, paralysis , or death.
