ACKNOWLEDGEMENT AND ASSUMPTION OF RISKS
I understand there are inherent risks associated with participation in the Educator Course, including but not limited to: exposure to the elements and nature in all of its forms, all aspects of performing rigorous physical activity individually and as a group,
use of training equipment, and engaging other activities designed to challenge and develop participants physically and mentally. Course activities have been described to my satisfaction.
I knowingly and freely assume all risks related to or arising from my participation in Educator Course activities, even if arising from the negligence of Iliad Athletics or other participants. I have no mental or physical impairments, injuries, health
conditions, or illnesses that will endanger me or others.
WAIVER AND RELEASE OF CLAIMS
In consideration of me being allowed to participate in the Educator Course, I RELEASE, DISCHARGE, AND AGREE TO DEFEND, INDEMNIFY, AND HOLD HARMLESS THE EDUCATOR COURSE SITE PROPERTY OWNER(S) AND ILIAD ATHLETICS, INCLUDING THEIR OWNERS, EMPLOYEES, VOLUNTEERS,
CONTRACTORS, AFFILIATES, AND AGENTS, FROM ALL LIABILITY, CLAIMS, AND CAUSES OF ACTION OF ANY KIND, FOR INJURY, INCLUDING DEATH, DISABILITY, OR ILLNESS, OR LOSS OF PERSONAL PROPERTY OF THE ABOVE NAMED PARTICIPANT RELATED TO OR ARISING FROM COURSE ACTIVITIES,
EVEN IF ARISING FROM THE NEGLIGENCE OF ILIAD ATLETICS OR OTHER PARTICIPANTS, INCLUDING COURSE RELATED TRAVEL. This release is also intended to include all liability, claims, and causes of action of any kind made by the participant’s family, estate, heirs,
personal representatives or assigns to the extent permitted by law.
PHOTO RELEASE AND CONSENT AGREEMENT
I hereby grant Iliad Athletics, LLC the right to use, reproduce, and/or distribute photographs, films, videotapes, and sound recordings of the participant identified above engaged in Educator Course activities for use in Iliad Athletics' marketing and promotional
materials.
