SOCIAL MEDIA AND CONSENT FORM

Purpose for this Form:

At Georgia Hair Solutions, we believe every client has a unique story, and we hope to inspire others by sharing these transformations. From time to time, we capture photos and videos of our clients' experiences, results, and testimonials for use on our social media, website, and other marketing materials.

Your comfort and privacy are our top priorities. Participating in photos and videos is completely optional- you are always welcome, whether or not you choose to be featured.

Please indicate your consent preferences below so that we may honor your wishes:

 

GEORGIA HAIR SOLUTIONS AND SALON FULL CONSENT RELEASE

I hereby authorize Georgia Hair Solutions and Salon, a Georgia corporation, any of its subsidiaries, and those acting pursuant to the authority of the foregoing (collectively, “Georgia Hair”) to:

1) Record my image, likeness, and voice in video, audio, audio-video, photographic, analog, digital, or electronic format, or any other copying or recording medium, still or moving (collectively the “Image”).
2) Use my name in connection with the Image, and combine the Image with other images, text, graphics, sound or other items.
3) Use, reproduce, crop, alter, modify, exhibit or distribute the Image in any medium (including, without limitation, the internet, web pages, social media platforms, emails, print publications, film, television, radio, video tapes, DVD, CD-ROM, and others) for any purpose that Georgia Hair deems appropriate, including promotional, marketing or advertising efforts.
4) Use, reproduce, publish, and otherwise utilize my personal story, testimonial, or narrative as it relates to the products and/or services offered by Georgia Hair.

I release Georgia Hair from liability for any violation of any personal, privacy or proprietary right I may have in connection with such use. I understand that all such recordings, in whatever medium, shall remain the property of Georgia Hair (or its applicable subsidiary). I have read and fully understand the terms of this consent and release, which contains the entire agreement of the parties.

To the extent applicable, I hereby agree to the foregoing provisions on behalf of my child under 18 years of age:

CLIENT ACKNOWLEDGMENT FORM 

Please review the following important information about our services and payment policies before your appointment.

Wig Shop and Salon are Seperate Services

At Georgia Hair Solutions and Salon, we offer two distinct types of services:

1. Wig Shop Services- includes wig consultations, fittings, and custom wig work or orders. 

2. Salon Services- includes haircutting, styling, wig washing, custom colors, and related hair care. 

These services are charged seperately and can be provided by different professionals. 

Wig Shop Policy

- A wig consultation fee is required for all wig appointments.

- Tips are appreciated but never expected.

- Any tips given for wig services go directly to the Wig Specialist.

- All wig- related payments are handled seperately from the salon services. 

Salon Service Policy

If you choose to recieve additional services such as:

- Haircut or trim

- Styling

- Wig wash and style

- Custom color

These are seperate salon services and require seperate payment to the hairstylist.

- The price is up to the discretion of the stylist, based on: Amount of hair cut, complexity of the service, time required, amount of product used, etc.

- Tips are appreciated but never expected.

- All fees and tips for salon services go directly to the hairstylist. 

- All services are final sale- there are no refunds on services provided.

 

Insurance:

We are NOT an insurance provider and do NOT file with insurance. We provide written receipts with our Tax ID Number on them after purchase is made. We cannot guarantee reimbursment from insurance. We take payment up front and client is responsible for further actions with insurance. 

 

Additional:

We specialize in Human hair wigs. Human hair wigs do NOT come in the color white. Our red human hair wigs are limited but we do have three options we love.
