2025 Gift of Hearing

Instructions: Please complete the form below to be entered into Momentum Hearing Aid Services’ Gift of Hearing Giveaway.

One individual in need will be selected to receive a free pair of premium ReSound Vivia hearing aids, along with a full diagnostic hearing evaluation and personalized fitting.

Provide as much detail as possible so our clinical team can accurately assess candidacy. All information submitted will remain confidential and used solely for the purpose of determining eligibility.

The more detail you can provide, the better we can understand their needs and determine candidacy. Feel free to include personal circumstances, communication struggles, or anything else that gives us a clear picture of who they are and why this gift would matter.

Disclaimer & Eligibility Requirements

Eligible entrants must be local to Central Pennsylvania and able to travel to our Harrisburg or Carlisle offices for evaluation, fitting, and follow-up care.

Submission of this form does not guarantee selection or confirm medical/clinical suitability for hearing aids. 

The selected winner must contact Momentum Hearing Aid Services to schedule a comprehensive hearing evaluation within 30 days of the announcement. The individual must meet clinical candidacy requirements and demonstrate measurable hearing loss to receive the Gift of Hearing devices.

Hearing aid technology included in this giveaway is limited to one pair of ReSound Vivia premium hearing aids; model and features are subject to availability.

No purchase is necessary.

Giveaway is void where prohibited. By submitting this form, entrants agree to be contacted by Momentum Hearing Aid Services for scheduling, eligibility review, and follow-up communications.

Selection will be based on clinical need and narrative provided in the application.

Winner must accept the prize within the specified timeframe or an alternate applicant may be selected.

The recipient of the Gift of Hearing will be announced on Facebook on December 24, 2025 at 5:00 PM.

All hearing aids provided through this giveaway are strictly for personal use and may not be sold, transferred, or used for resale purposes.
