  • Please press NEXT to continue the rest of the consent process.

  • Important Reminders:

    • Bring a responsible adult with you who has signed the consent form. While they
do not need to stay for the entire appointment, they must be reachable by phone.
Ideally, we would meet them before beginning your treatment.

    • Plan for rest. Do not schedule any important tasks or responsibilities for the
remainder of the day after your appointment.

    • Provide accurate medical history. It is essential to disclose all medications and
any recreational drug use, as these can impact your sedation. We are nonjudgmental but require full transparency for your safety.

    • Relax! Our goal is to make your experience as smooth and stress-free as possible.

If you have any queries at all about the sedation, please get in touch and we’ll be
sure to answer any questions you may have and put your mind at ease.

  • Medical History Form

Although dental personnel primarily treat the area in and around your mouth, your
mouth is part of your entire body.

Health problems that you may have or medications that you may be taking could, have an important interrelationship with the dentistry you will receive.

  • Please answer the following questions as honestly as possible.

Tick appropriate box and please list and further details in the
    space provided.

  • Examples of unit measurements:

A pint of average-strength (4% ABV) lager contains about 2 units.

A single measure (25ml) of spirits (40% ABV) is 1 unit.

A 125ml glass of typical-strength champagne has around 1.5 units.

A standard glass (175ml) of Red Wine has around 2 units.
