• Missions Trip Application Form - July, 2026

    2026 Mission Trip will be to the Czech Republic in support of Al and Stacy Anderson’s Young Life Ministry.
  • Thank you for your interest in being a part of the Edgewood outreach to our missions. We are excited that you are considering being a part of one of our international mission teams. We believe that these types of international serving opportunities are a foundational tool that God uses in helping Christ followers grow in their commitment to Jesus. We will be praying for you as you discern if this is an opportunity that God is inviting you into.

  • What requirements are there to be a part of the team?

    We ask that you consider Edgewood your church home as a regular attender. In addition, all training, and debrief meetings are MANDATORY, as are fund-raising events. There will be 5-6 meetings before leaving and a debrief meeting AFTER you return from the outreach trip.

  • ALL of the following items must be submitted in order for your application to be considered complete:

    1. Complete this International Mission Application

    2. Bring copy of current valid US passport to first meeting
      (if you do not have a passport, please share your plan to get one)

    3. Signed *Immunizations information document
      *There are no Special immunizations required for the Eastern Europe trip with exception that you may want to ensure you have an updated tetanus vaccine.
  • Edgewood Missions Team plans international outreach trips as God leads and provides a team passionate about the work. Please take time to complete this application so that we can consider if this opportunity is a good fit for you. We encourage you to always seek God’s will first, and to prayerfully consider any desire to do outreach work. A completed application is required for participation on every international mission team, regardless of previous experience.

  • Becoming a member of an International Mission team with Edgewood Church requires a certain level of sacrifice and commitment. In addition to completing the required training and orientation program, there are pre-trip fund-raising events, planning meetings and a post trip debriefing that will require your mandatory attendance.

    It is important that you acknowledge the fact that there will be cultural differences on these trips, and that you commit to doing your best to be a witness for Jesus Christ in the host culture without judging or condemning. Please read the following Team Covenant carefully and affirm your commitment by initialing each statement and signing below.

  • As a member of this team, I agree to the following (check next to each statement):

  • Spiritual Growth:

    I will be open to learn from the Holy Spirit in all situations. I will complete the required training so that I am prepared for service on this mission field.

  • Teamwork:

    I commit to working for team unity at all times. I commit to strive to be a servant to those on my team.

  • Team Meetings (Pre-trip, trip, & post-trip): I commit to participating in all team meetings before, during and after the trip. I commit to being prepared for all team meetings, including completing all assigned readings and exercises.

    I understand the value of team meetings for discipleship, training, team building, logistics, and debriefing.

  • Positive Attitude:

    I agree to have an open spirit of love and encouragement towards my fellow teammates and towards those we are serving.

  • Ministry and Work Opportunities: I will strive to be a Christ-like example to those I come in contact with throughout the trip. I commit to working hard at all tasks, sharing my faith and testimony as the Lord leads.

  • Health and Personal Risk:

    I commit to taking responsibility for my own health throughout my entire time on the outreach trip. I will not take risks which jeopardize my own or the health, safety or convenience of others.

  • Leadership:

    I agree to accept and to submit myself to the appointed leadership of the team.

  • Guest:

    I acknowledge that I am an invited guest of our host ministry and culture. I agree to refrain from making any negative comments or engaging in any disagreement about cultural differences, people, politics, sports, religion, race or traditions.

    I acknowledge that Christianity looks different in other parts of the world and my approach on this trip will be one of learning about what the Global Church looks like in this particular community and people group.

  • Relationships:

    I agree to be sensitive in how I relate to members of the opposite sex. I will not pursue relations beyond friendship.

  • Witness:

    I agree to refrain from the use of any and all tobacco products, any and all alcoholic beverages, and all illegal substances during the course of the trip and at any team gatherings.

  • Prayer and Finances:

    I commit to raise the necessary prayer and financial support for this trip. I will support the team efforts for fund-raising and share jointly in their success.

    I understand that my payments must be made on the dates set by the team leader(s) and that ultimately my trip cost is my complete and personal responsibility.

    If I am unable to raise or pay for my trip cost, the church is NOT responsible for paying the amount due.

  • I have read and understand all of the above, and I commit to abide by these requirements. I understand that if I fail to meet my commitment to this covenant, I may be asked to step down from the team. I commit to conduct that is above reproach and representative of my faith in Jesus Christ.

  • PASSPORT INFORMATION

    To apply for a U.S. passport, you will need:

    • An original proof of citizenship document
    • An acceptable photo ID document
    • A photocopy of the front and back of the photo ID document
    • Passport photo (two copies can be taken at local US Post Office or AAA)

    Allow 6 months for processing time before the trip and the current expiration date must be at least 6 months after trip.

  • IMMUNIZATION INFORMATION

    Each region requires different immunizations. For the most accurate list of requirements, please refer to your physician and the Centers for Disease Control and Prevention website.

    CONTACT YOUR PHYSICIAN –
    It is always highly recommended to speak to a physician as soon as you know you will be traveling. It is especially important to consult a doctor if there are any health conditions or concerns that you may have.

    TIMING OF IMMUNIZATIONS IS CRUCIAL!
    Your physician will provide information on immunization schedules (best time to begin the shots, etc). Waiting too long to get immunized may prevent you from traveling!
