ALL AMERICAN TWIRLERS Waiver of Liability & Release Form. I am the Parent/Guardian Acknowledgment of Risk I understand and acknowledge that baton twirling, physical conditioning, and related activities involve inherent risks, including but not limited to: falls, collisions, muscle strains, sprains, injuries from dropped equipment, and other potential hazards associated with athletic activities. I voluntarily assume all risks associated with my/my child’s participation in All American Twirlers practices, events, performances, and related activities. 2. Release of Liability In consideration for participating in All American Twirlers programs, I hereby release, waive, and discharge All American Twirlers, its owners, coaches, instructors, staff, volunteers, and facility providers from any and all liability, claims, demands, causes of action, or damages arising from injury, illness, or loss sustained during participation.3. Medical AuthorizationI authorize All American Twirlers staff to administer basic first aid and, if necessary, to seek emergency medical care for me/my child. I understand that I am fully responsible for all medical expenses related to such care.4. Photo/Video Release (Optional)I ☐ DO / ☐ DO NOT grant permission for photos or videos of my child to be used for promotional purposes on social media, websites, or printed materials for All American Twirlers. 5. Behavior & Participation AgreementI understand that all participants must demonstrate respectful behavior, follow coach instructions, and maintain a safe environment. Failure to do so may result in dismissal from the program without refund.6. Payment & Refund PolicyI understand that all registration fees, tuition payments, and associated costs are non-refundable. Missed practices will not result in credit or makeup classes unless otherwise stated by All American Twirlers.---I have read, understand, and agree to the terms of this wavier and release.
