Ultimate Leader Scholarship Application
The Ultimate Leader Scholarship supports Nebraska FCCLA members enrolling in a postsecondary institution with any major other than Family and Consumer Sciences Education. To be eligible to apply, applicants must:
- Be a member of an affiliated Nebraska FCCLA chapter for at least one full year.
- Be actively involved in their chapter’s activities.
- Have completed at least one semester (or the equivalent) of Family and Consumer Sciences classes.
- Be a graduating senior with a cumulative GPA of at least 3.4 on a 4.0 scale (85% average).
- Meet at least one of the following criteria:
  1. Currently hold or have served as an FCCLA Chapter Officer, District Officer, State Peer Officer, State Officer, or National Officers.
  2. Currently or previously competed in any District, State, or National FCCLA Competitive Events.
  3. Currently or previously completed a National Program project.
In order to complete this applcation, applicants must also upload their Resume, which includes:
- FCCLA participation at the local, district, state, and national level
- FCCLA officer positions held
- FCCLA recognition received
- School participation & involvement
- Community participation & involvement
These are the minimum requirements for the Resume; additional categories & information can be included.

Once you submit your scholarship application through Jotform, your chapter adviser will automatically receive a Scholarship Materials Submission Form. This form is where your official transcript and the required two (2) Recommendation Letters will be submitted on your behalf.

Your chapter adviser is responsible for collecting and uploading the following materials in the Scholarship Materials Submission Form:
- Applicant's Official Transcript
- Two (2) Recommendation Letters
  - FCCLA chapter adviser
  - Adult familiar with the applicant's qualifications
If the applicant's chapter adviser is also their parent, select another adult school representative to write the recommendation letter.
