    • PART 2: AGREEMENTS

    • At QL2 Dance, we are committed to providing a safe, creative, and inspiring environment where you can develop your skills, express your creativity, and be part of a supportive dance community.  

      This  part of the form outlines guidelines for participation, and agreements for you to sign. Please read through this section carefully to ensure you understand and agree to the terms of participation. If you have any questions, please be in touch.  

    • 1. Attendance and Participation  

    • Attending class consistently will ensure the most efficient development for you as a dancer. We encourage you to attend class every week and participate fully.  
        
      Attending classes is not just about developing your dance skills. Being active, belonging to a community and having a sense of purpose all contribute to good mental health. We encourage you to commit to classes for yourself, and as part of the QL2 community. For further information on the benefit of arts for mental health, visit this link.  
        
      We encourage you to prioritise your dance classes as part of your regular routine, even when you are busy. Learning how to manage your time is another important life skill. When you have school or university deadlines, moving your body will give you a break and allow you to process information in different, perhaps deeper ways.  
        
      What if I am injured, or not feeling well?  
      Learning how to participate when you are not feeling your best is a good life skill. Often, it is still possible to participate in class, in a modified way. QL2 staff will support you to do this, when you need it.  
        
      If you are unwell:  

      1. If you have active symptoms – including vomiting, diarrhoea, coughing, sneezing – please stay at home to minimise the risk of passing on your illness to others.  
      2. If you are feeling unwell but you are not symptomatic, we encourage you to attend and participate as best you can. Let your teacher know how you are feeling so they may help you adjust class content as required. Your participation may involve observing at times.  
      3. If you become unwell during class, your teacher may direct you to the QL2 staff member on site for support. If you are unable to continue, we may contact your parent/guardian to collect you before the scheduled end of class.  

      If you are injured: 

      1. If you arrive at class with an injury, ensure this is communicated to QL2 staff and your teacher, before class begins. Your teacher may help you adjust class content as required. Your participation may involve observing at times. 
      2. If something happens during class, which impacts your ability to participate fully, ensure this is communicated to your teacher, and QL2 staff.  
    • 2. Health and wellbeing  

    • To support your health and wellbeing, QL2 Dance agrees to: 

      1. Facilitate professional dance experiences for you and support your development as a young dancer within this environment - with information, explanations and open dialogue. 
      2. Create and maintain a safe working environment for you. This includes providing working spaces with dance-safe flooring to support dynamic movement. 
      3. Provide a thorough warm up at the commencement of each class. 
      4. Facilitate an inclusive and respectful working environment, and methodology.  
      5. Provide clear direction, instruction and feedback where appropriate.  
      6. Be available for discussion.

    • Please note: QL2 programs are not currently resourced to provide ongoing one-on-one support for participants. If your young person requires a higher level of support than we can provide, we will be in touch to discuss this with you.  

    • 3. Image Release

    • From time to time, QL2 engages professional photographers and/or videographers to capture our programs and participants.  
        

      I hereby give my consent for QL2 Dance to record, copy, edit, adapt, modify, distribute or exhibit my image, likeness, voice and/or transcript, in whole or in part, in any form or media, and including the right to sublicense these rights for educational and promotional/publicity purposes for an undefined period of time. I understand that I will not receive any payment or consideration in agreeing to this release.

    • 4. Venue Security

    • Our home at Gorman Arts Centre is located on a public thoroughfare adjacent to the Canberra CBD.   

      To ensure the safety of all staff, volunteers and participants, QL2 Dance has put in place the following requirements:  

      • Shoes must be worn when arriving, leaving and moving between spaces at Gorman Arts Centre. 
      • The front door of either the Studio or A Block may be locked during class times. 
      • QL2 Dance recommends accompanying your young dancer to and from the front door of the venue. Please be aware that QL2 staff are not available to accompany dancers around the venue to car parking areas.
    • 5. Child Safety

    • QL2 Dance commits to providing a welcoming and supportive environment for young people to explore dance and artistic expression. This includes promoting child safety and protecting all young people’s best interests.  

      QL2 values diversity of all kinds. We have a zero-tolerance policy regarding discrimination against any child (or adult) based on their gender, race, religious beliefs, age, ability, sexual orientation, or family or social background.   

      All QL2 staff, artists, teachers and volunteers adhere to our Child Safety Policy. This agreement aims to nurture a culture that promotes children's safety, wellbeing and participation.  

      Any actual or suspected inappropriate behaviour or abuse should be reported to our Child Safety Officers:

      QL2 Dance also commits to seeking the views and experiences of our community through open discussion, surveys and consultation.  

    • PART 3: TERMS AND CONDITIONS

    • You are registering as a casual participant on a term-by-term basis.
        
      CHANGE OF MIND  
      We do not provide refunds for change of mind.  
       
      Please note, young dancers may be invited to shift levels, if/as recommended by QL2 teaching artists and/or staff during the year. On acceptance, this would alter your weekly schedule.  

      INJURY OR ILLNESS – SHORT TERM  
      Please refer to Part 2 of this form: Health and Wellbeing regarding Attendance and Participation. 
        
      INJURY OR ILLNESS – ONGOING   
      If your attendance is severely impacted during the year due to ongoing injury or illness, please provide QL2 Dance with a medical certificate, and arrange an appointment to discuss.

    • PART 4: CONSENT

    • This document is a legally binding agreement between The Dancer or their legal guardians, and QL2 Dance Incorporated ABN 78 745 535 672. 

    • I/we:  

      • Consent to the Participant being under the authority and care of QL2 Dance and its employees and/or delegates. 
      • I consent to the information provided in this form being made available to QL2 staff, volunteers and others QL2 may delegate, if and when appropriate. 
      • Expressly assume the risk of participating in programs with QL2 Dance. I/we release and agree to indemnify and hold harmless QL2 Dance and its officers, agents, and staff from any and all claims resulting from or arising out of my participation in the program including without limitation all claims, demands, causes of action, suits, losses, or damages due to my injury or death during my participation. 
      • Agree that QL2 may determine, in its absolute discretion, whether the Participant may commence or continue in any activity; and agree that QL2 may remove the participant from programs if the Participant acts significantly contrary to this Agreement. 
      • Consent to QL2 making decisions or performing actions which QL2 in its absolute discretion judges to be necessary or desirable for the welfare of the Participant or other participants. 
      • Agree that, while QL2 will attempt to contact us prior to any emergency action, QL2 is empowered to act in its absolute discretion whether or not such contact is successfully made.   
      • Agree to provide any medication required or advised for the Participant, together with instructions for its use by the Participant and documentation where appropriate from a medical practitioner; and accept that while we may notify QL2 of health issues or medication for the Participant, QL2 does not undertake to manage or treat health issues, maintain a treatment plan, or administer medication.
