    Medical Consent

    I confirm the participant is fit to take part. I authorise staff to provide or seek medical assistance if required, and I accept responsibility for any associated costs.

     

    Liability Release

    I release Groove Dance & Cheerleading, its owners and staff from liability for injury, illness, loss or damage that may occur during classes or on studio premises, except where caused by negligence.
