·         It is acknowledged and accepted by the signee that that participation in contact sport, including football, carries an inherent risk of injury that may result in permanent disability or death and/or damage to property. Such injuries or damage may arise through, but not limited to, contact with other participants whether or not in accordance with the rules of the game, match officials, the ground or equipment. Due to their inherent nature, the signee accepts that such risks cannot be wholly removed or mitigated.

·         The signee accepts that participation in the event is entirely voluntary and elects to participate notwithstanding the risks acknowledged above.

·         The signee confirms that they have satisfied themselves that the playing conditions, including the pitch, are safe.
