PARTICIPANT WAIVER, RELEASE OF LIABILITY, & MEDIA CONSENT
Event: Big Baller Brand Presents – Dayton Unity Skills Clinic
Dates: December 13th & 14th, 2025
Location: 730 South Alpha Bellbrook Road, Sugarcreek, Ohio 45305
Host Partners: Big Baller Brand, Triple B’s Cares Foundation, Dayton Muslim Basketball Organization
1. ASSUMPTION OF RISK
I, the parent or legal guardian of the participating child, understand and acknowledge that participation in a basketball camp involves physical activity, movement, exertion, and inherent risks, including but not limited to:
Sprains, strains, injuries, accidents, or collisions
Dehydration or fatigue
Other risks associated with athletic activity
I fully understand these risks and voluntarily allow my child to participate.
2. RELEASE OF LIABILITY
By signing below, I agree to waive, release, and hold harmless the following parties from any and all liability, claims, demands, or actions arising out of or related to my child’s participation in this camp:
Big Baller Brand Inc.
Triple B’s Cares Foundation
Dayton Muslim Basketball Organization
All event staff, coaches, trainers, employees, partners, contractors, and volunteers
The event venue and its owners, operators, and representatives
This release applies to injuries, accidents, illnesses, or damages of any kind that may occur during or as a result of camp participation.
3. MEDICAL CONDITION STATEMENT
I confirm that my child does not have any severe or undisclosed medical conditions that Big Baller Brand, the Dayton Muslim Basketball Organization, or event staff need to be aware of.
If my child has allergies or medical needs, I understand it is my responsibility to notify camp staff in writing before the camp begins.
4. MEDICAL AUTHORIZATION
In the event of an injury or emergency, I authorize camp staff, trainers, or volunteers to obtain basic first aid or emergency medical care for my child.
I understand that I am solely responsible for any medical expenses that may result.
5. PARENT DROP-OFF & PICK-UP RESPONSIBILITY
I acknowledge that it is my full responsibility to ensure that my child is:
Dropped off safely
Picked up promptly at the end of their designated session
Big Baller Brand and its partners are not responsible for participants outside of the scheduled camp hours.
6. MEDIA RELEASE
By signing below, I grant Big Baller Brand, Triple B’s Cares Foundation, Dayton Muslim Basketball Organization, and all affiliated partners the full right to:
Photograph
Record video
Use my child’s image, voice, name, or likeness
for promotional, marketing, social media, livestream, and educational purposes, without compensational obligation.
I understand that all media captured becomes the exclusive property of Big Baller Brand and its partners.
7. AGREEMENT & SIGNATURE
By signing this form, I acknowledge that:
I have read and understood all terms outlined above.
I accept full responsibility for my child’s participation.
I voluntarily agree to all conditions, waivers, and releases.
My signature indicates full consent and authorization for participation.
