Description/Instructions:

By completing this section, you authorize Southern California Junior Cricket Academy to capture, use, and publish photographs, video recordings, or digital media of you (or your child, if applicable) for educational, promotional, and documentation purposes.

This may include use on websites, social media platforms, printed materials, and other media formats.
Your participation is voluntary, and you may revoke your consent at any time in writing.
