Ownership Release Agreement* This field is required.I hereby certify that I am the lawful owner, keeper, or caretaker of the animal(s) listed in this Animal Surrender Form, hereafter referred to as “the animal.” I certify that no other individual or entity has any legal or ownership claim to the animal. I voluntarily and irrevocably surrender all property rights and ownership interests in the animal to Abigale Choi / Your Local Fur Mama, a Tennessee-based rescue organization. I understand and agree that once surrendered, the animal may be placed with another rescue, shelter, or transport partner and will be rehomed at the sole discretion of Your Local Fur Mama. I certify that the animal has not bitten or caused injury to a human or another animal within the past 10 days. I understand that once I relinquish the animal, the animal cannot be returned to me under any circumstances. By signing below, I confirm that I have read, understand, and agree to all terms of this Animal Surrender Form.
