This agreement is made by and between the undersigned Lab Participant and Axis Research & Technologies. This Agreement will remain
on file and is in effect for the duration of three (3) years from the date signed below.
1. Lab Participant agrees to wear appropriate attire and personal protective equipment (PPE) at all times while participating in the bioskills
lab event. This includes, but not limited to , scrubs, and/or gown, gloves, closed toe shoes, shoe covers, and eye protection.
2. Lab Participant represents that he/she has been trained in Universal precautions as defined by the Occupational Safety and Health
Administration (OSHA) laws and regulations and agrees to follow the universal precautions and any other lab and safety precautions
implemented by Axis Research & Technologies. All hazardous material and sharps shall be disposed of properly.
3. Axis Research & Technologies has certified that all tissue specimens provided to Lab Participant have been minimally screened and
serologically tested for Human Immunodeficiency Virus (HIV) types 1 & 2, Hepatitis B surface Antigen (HbsAg), and Hepatitis C Virus
(anti-HCV). NO OTHER diseases have been tested or screened for unless otherwise specified in writing by COURSE PROVIDER.
4. Axis Research & Technologies/RFL will not release tissue that has tested serologically as confirmed reactive. All other tissue may be
released at the sole discretion of Axis Research & Technologies.
5. Lab Participant is fully aware that handling of human tissue is a hazardous activity.
6. Lab Participant agrees to release and hold harmless Axis Research & Technologies, it’s owners, officers, directors, employees, agents,
contractors, sponsor of this bioskills event and affiliates from any and all liability, loss, claim, damage or injury that might arise or result
from, or relate to , now, and in the future, Lab Participant’s contact with tissue specimens while participating in this lab course.
7. Lab Participant agrees to release and hold harmless Axis Research & Technologies, it’s owners, officers, directors, employees, agents,
contractors, sponsor of this bioskills event and affiliates from any and all liability, loss, claim, damage or injury that might arise or result
from, or relate to , now, and in the future, Lab Participant’s contact with or use of all equipment and instrumentation including scalpels,
drills, saws and exposures to x-rays originating from the lab or lab activities.
8. Lab Participant understands that it is the policy of Axis Research & Technologies to treat donated human anatomical specimens with
the utmost respect and gratitude at all times, and the Lab Participant pledges to comply with this policy.
9. Lab Participant understands that the unauthorized use of photographic, video or other electronic recording equipment in lab events
supported by Axis Research & Technologies in rooms containing human tissue is expressly prohibited.
10. Lab Participant acknowledges that any pre-existing intellectual property owned or licensed by Axis Research & Technologies and to
which the participant is exposed during the facility usage shall remain the exclusive property of the Axis Research & Technologies.
Participant agrees to treat any intellectual property, process, or confidential information of Axis Research & Technologies as
confidential and not to disclose, reproduce, or use such information for any purpose other than the agreed-upon facility usage.
11. Media The marketing team at Axis Research & Technologies plans to use photographs/videos to enhance its social media presence.
These photographs may include images that feature our employees, patrons, Axis facilities and activities within our facilities. Axis
Research & Technologies would like to state that although the photos may be used, we will not release any names or details about the
individual featured. These images will greatly contribute to the success of showcasing our company and what we do.
Lab Participant has read this Agreement and understands its content. This Agreement with Axis Research & Technologies/RFL is being
signed by Lab Participant’s own free will.
