The customer identified above affirms that they are of legal age and eligible to receive salon services, including but not limited to haircuts, hairstyling, hair coloring, hair lightening, hair extensions and other related services.

By signing this form, the customer acknowledges that they have received and understand all information, suggestions, and recommended services provided by the hairstylist Britney Hill and the salon Hair by Britney. The customer agrees to proceed voluntarily with the selected services.

The customer hereby releases the salon Hair by Britney and hairstylist/salon owner Hair by Britney from any liability, claims, damages, or demands that may arise from injuries, damages, or losses associated with the services provided.

A 20% deposit is taken to secure your appointment and will be applied to the total cost on your scheduled appointment day. A 48-hour cancellation notice is required for all scheduled appointments. If the client fails to provide notice at least 48 hours prior to the appointment time, or in the event of a no-show, the 20% deposit will be forfeited.

All sales are final, and no refunds will be issued for completed services or forfeited deposits.
