Homestead Christmas Workshop Waiver and Release Form. To ensure a safe and enjoyable experience for everyone, we kindly ask you to read and complete the following waiver and release form. This form is required for all participants, both children and adults. Waiver and Release of Liability Acknowledgment of Risk: I, the undersigned, understand that participation in the Homestead Christmas Workshop involves certain inherent risks, including but not limited to the use of hand tools, paints, and other materials. I acknowledge that these activities can result in personal injury or damage to property. Release of Liability: In consideration of being allowed to participate in the Homestead Christmas Workshop, I hereby release and discharge The Self Reliance Homestead, its instructors, volunteers, and any associated entities from any and all claims or causes of action, known or unknown, arising out of any injury, loss, or damage that may occur during my participation in this class. Parental Consent (for participants under 18)I, the undersigned, am the parent or legal guardian of the minor participant. I have read and understand this waiver and release form and hereby consent to the terms stated herein on behalf of my child. I agree to indemnify and hold harmless Scott and Katie Burleson, The Self Reliance Homestead and its representatives from any and all claims arising from my participation in the class.
