Please fill out this form to authorize your child to participate. One form is sufficient for all three events.
TRANSPORTATION LIABILITY WAIVER - My son/daughter has my permission to ride to and from SonRise Bible Church in Atkinson, IL and the following locations on the following days:

May 3, 2026 - Miniature Golf - Indian Creek Mini Golf @ 780 E Culver Ct, Geneseo, IL 61254

June 7, 2026 - Swimming Pool Party - Kale & Katie VanAntwerp's home @ 16180 N 2120 Ave. Geneseo, IL 61254

August 2, 2026 - The Fun Station Quad Cities, 400 S 14th Ave, Eldridge, IA 52748

I do release SonRise Bible Church, their ministers and volunteer leadership, from liability for injury or accident, and do give my permission to the church’s leadership to administer basic first aid and secure proper medical attention should the need arise.

PHOTO RELEASE WAIVER - I also grant SonRise Bible Church permission to use photographs and/or video taken of my son or daughter at, in the vehicle to and from the event, and while at the event.
