Youth Bowling Event Waiver

Please fill out this form to authorize travel for your child.
TRANSPORTATION LIABILITY WAIVER - My son/daughter has my permission to ride to and from SonRise Bible Church in Atkinson, IL and Malibu Jacks in Bettendorf, IA for a youth bowling event on December 7th, 2025. I do release SonRise Bible Church, their ministers and volunteer leadership, from liability for injury or accident, and do give my permission to the church’s leadership to administer basic first aid and secure proper medical attention should the need arise.

PHOTO RELEASE WAIVER - I also grant SonRise Bible Church permission to usephotographs and/or video taken of my son or daughter at, in the vehicle to and from the event, and while at the event.
