Golf Simulator Liability Waiver and Release Form

I acknowledge that participation in the golf simulator activity involves certain risks, including but not limited to physical injury from swinging golf clubs, accidental contact with equipment, and other unforeseen hazards. I voluntarily assume all risks associated with my participation and agree to release and hold harmless The Bunker, its owners, employees, and agents from any and all liability, claims, demands, or causes of action arising out of or related to my participation in the golf simulator activity.

I understand that the golf simulator activity requires physical exertion and coordination, and that injuries such as muscle strains, bruises, or other physical harm may occur. I certify that I am physically fit and capable of participating in this activity and have not been advised otherwise by a qualified medical professional.

I agree to inform the staff of any medical conditions, allergies, or medications that may affect my ability to safely participate in the golf simulator activity. I understand that it is my responsibility to manage my health and safety during participation.
