Notes and Procedures
1. This form may only be completed by a registered veterinarian, a GSDFSA Breed Judge, or a GSDFSA Recognised Tattooist. If completed by anyone else, this form will be disregarded.
2. The form cannot be completed by any person who is registered as the owner or co-owner of the dog mentioned.
3. A singular form must be completed for each dog. More than one dog cannot be added to the same form.
4. Use this form to declare the microchip number of a specific dog, where the Federation only has a Tattoo Number on record. This form can be used for dogs being microchipped to be radiographed for HD/ED, or for dogs that have previously been microchipped, but where the microchip is not on record with the Federation.
5. Puppies that have been registered with the microchip number and not a tattoo number, or where the microchip number was provided to the GSDF offices upon registration of the litter, and checked by the tattooist, do not need to have this form completed.
6. Completing this form does NOT substitute registering the microchip on a public database, and we still recommend registering your microchip on the provider's website/database.
7. We urge vets or any party that microchips a GSDF-registered dog to complete this form immediately upon microchipping a dog, to keep the GSDF records up to date.
8. Only 15-digit Microchips may be used.
